We utilize an Advanced Care Team model as an intregal part of our medical home.  Advanced Care Team members support the Primary Care Provider (PCP) in efforts to deliver effective patient centered care.

We are proud that Raelene, LPN, our Care Team Manager has been with Coastal Children's Clinic for over 20 years supporting your provider to deliver effective patient centered care.  Our advance care teams are provided to all patients. The team coordinates and works together to support our patients:

  • facilitates communication between child, parent and provider
  • helps with decision making
  • promotes quality of care
  • triage services
  • promotes continuity of care

Dental Health

We will prescribe oral flouride supplements starting at age 6 for children whose water supply is deficient in flouride.  We will also apply a flouride varnish to primary teeth of all infants and children starting at age 6 months when covered by insurance.
