We utilize an Advanced Care Team model as an intregal part of our medical home.  Advanced Care Team members support the Primary Care Provider (PCP) in efforts to deliver effective patient centered care.

We are proud that Raelene, LPN, our Care Team Manager has been with Coastal Children's Clinic for over 20 years supporting your provider to deliver effective patient centered care.  Our advance care teams are provided to all patients. The team coordinates and works together to support our patients:

  • facilitates communication between child, parent and provider
  • helps with decision making
  • promotes quality of care
  • triage services
  • promotes continuity of care

We have recently joined forces with The Kids in Parks TrackRx.  Get Unplugged.  Get Outdoors.  Get prizes. The Kids in Parks was created to provide kids and families with fun outdoor recreation opportunities.  Through their TRACK Rx program, we are promoting this program and formally prescribing our patients to get active outdoors and reconnected to nature through this exciting program.
